This is a form page custom post type (type of page). This is to show an example of how to create a page to host an adhoc form in Gravity Forms. We have created a form below representing an adhoc need and embedded it here in the Form Pages CPT as a page. This keeps this nice and organized within one section for hosted forms.

When a user submits the form below, the data is stored as an entry (accessible through Forms->Import/Export Entries) and you can export and leverage the data as you see fit.

The instructions to do this are:
1) Create the form you want with the appropriate fields.
2) Create a Form Page “page” and embed the form using the “add form” button
3) Publish the page and share the link or add it to the menu, etc.
